Case Management

Effectively caring for residents is complex. At Mattanja, a monthly case management program for residents monitors both their clinical and social outcomes so that we are better equipped to give them the care that is needed.  An organized approach of regular assessments of our residents avoids crisis management, and improves the quality of the resident’s experience at Mattanja.

What is Case Management and how can it be beneficial?

Case or care management helps ensure that the specific health and well-being needs and goals of the elder receiving care are understood, with specific strategies in place to achieve these needs and goals. The Care Giver not only helps to develop the care plan together with the elder, and his or her family, but also monitors how things are going to ensure that the correct strategies are in place and that the care and support provided is at the right level. Case Management is also a support service for clients and their carer or family. Whether this support is understanding the health information provided to them by a doctor or nurse, or what a diagnosis means, or finding ways to ensure that your loved one can live at home for as long as possible, a caregiver works pro actively to meet the needs of the elder. These might be changing needs due to having a cold or flu, longer term changes to aid with rehabilitation or a seasonal change like adding in extra social support around the Christmas holidays.
